Subjective assessments are "dependent on the mind or on an individual's perception for its existence." In other words, subjective information is based on what the patient feels and thinks.

Documentation is key! Properly documenting all subjective responses can help form a guideline for the patient's plan, and can also be a good tool in showing a patient how far they have come since the initial injury. Documentation is also necessary for insurance and legal purposes.
